Eligibility Forms

Welcome to Linn-Benton Community College Athletics. The following information is required in order for you to be eligible to participate in athletics at Linn-Benton Community College; however, completing and submitting all of the paperwork does not automatically deem you eligible to compete in athletics. You must meet all NWAC eligibility requirements before you are eligible to play. The Athletic Office will let your coach know when you are eligible to play. 

All student-athletes must complete required forms before participating. These include eligibility, insurance, health history and injury tracking forms.

From the LB Library

Welcome back! As Fall Term starts, the Library wanted to share some quick, essential information about our services and resources. Although Library facilities will remain closed for all of Fall term, virtually all of the library’s typical services are back up and running.

Key Contacts

  • For research assistance and general questions about library services and resources please contact the Reference Desk at (541) 917-4645 or libref@linnbenton.edu.

  • Students who need technical assistance with MyLB, Moodle, Gmail, Zoom, and other college software or with library equipment should contact the Student Help Desk at (541) 917-4630 or student.helpdesk@linnbenton. edu.

Laptops, Hotspots and other library equipment

  • Students can find information about requesting the loan of a laptop in the "Access to Library Laptops and Internet Hotspots" box on this page.

  • If students can't afford the technology or course materials they need for the whole term, it's best if we can help them get their own copy or device! Please refer them to the Roadrunner Resource Center (resources@linnbenton.edu). The FT Faculty Association Emergency Grant (willihm@linnbenton.edu) is another option.

Books, DVDs, and other library materials

  • Most of the library’s resources are electronic and can be accessed from the library’s website. If you can’t find what you are looking for or if you need to request an item in electronic format, please contact the Reference Desk or your favorite librarian.

  • Items from the Library's regular collection (the Stacks) and from our partner libraries in the Linn Libraries Consortium can be placed on hold and picked up at the Campus Store's curbside location once you've been notified they're available.

  • The Library can't currently offer our standard course reserves service for textbooks. 

    • Students can request up to 30 pages of scanned materials in our collection (including reserves) through this form.

    • If you have questions about posting readings or videos to Moodle, you can talk to a librarian about this.

  • Interlibrary loan service is also available. If you have questions about ILL, contact Fran Beck (beckf@linnbenton.edu).

Mental Health Resources

To access counseling services, call the LBCC Advising Center at 541-917-4780. During business hours, you will speak with our skilled reception team who will either connect you with the next available LBCC Counselor or will be transferred to our crisis counseling service, depending on the need. If you reach our voicemail box, you can always press 2 to be connected directly with a crisis counselor. The crisis service is also able to provide consultation to faculty and staff who may be seeking assistance in how to handle situations with students.

NCAA Two-Year Transfer Information

The NCAA transfer page contains updated versions of the NCAA Guide for Two-Year Transfers and the Eligibility Center’s Two-Year Transfer Best Practices Flyer, among several other transfer-related resources and flowcharts.

The Best Practices Flyer linked above provides detailed information on what students need to do ahead of their transfer. For additional help you can contact clarkin@ncaa.org or 317-917-6008 to discuss questions and receive feedback on any issues you are facing.

Propspective Student-Athletes

You can complete our prospective student-athlete recruiting form here. Your information will be sent to the head coach of that sport. We also encourate you to reach out directly to the head coach. You can view our staff directory here.
